Although this lesson’s resources all told the importance of the IEP being created by a team after gathering present levels of performance and soliciting the input of all team members, you will apply what you have learned about IEPs to the creation of an IEP for a fictitious student, Toby.
For this assignment, begin by locating an IEP form from your state or school district. You will then read the case study found in the lesson’s resources to create an IEP for Toby. Your IEP should address, at a minimum, the following components:
· Present levels of academic achievement and functional performance (PLAAFP)
· Measurable annual goals
· Reporting of student progress
· Accommodations, modifications, and support services
· Least restrictive environment and related services
· Participation in state and district-wide assessments
· Frequency and duration of services
· Transition services, if applicable
Then, at the end of your IEP, write a short narrative in which you predict potential legal issues and requirements that may arise during Toby’s IEP meeting. Use your understanding of case law and details from the Toby case study in making these predications. Propose ways that you might address the possible issues.
Length: 2-3 pages not including the completed IEP form, title, and reference pages.
